How to Acquire the Blood Moon Breeches
To secure the Blood Moon Breeches Unique item, players should focus on activities in World Tier 3 and World Tier 4. Unique items are most effectively farmed through high-level content such as Nightmare Dungeons, Gathering Legions, Helltides, and World Boss battles.
Introduced in Diablo 4 Season 2, the Blood Moon Breeches are part of the Uber Boss loot pool. This means that fighting Uber Bosses increases your chances of obtaining these Uniques. Specifically, you will need to battle either Duriel, King of Maggots, or the Echo of Andariel. Both of these Uber Bosses are found exclusively in World Tier 4 and require special summoning materials dropped by other Uber Bosses. Below is a breakdown of the required materials and locations:

Defeating the Tormented versions of these bosses provides the best chance for Unique drops, though they are significantly more challenging. The Tormented bosses require Stygian Stones, which can be obtained by completing The Pit runs—one of the more demanding activities in the game.
Effects and Affixes of Blood Moon Breeches
Equipping the Blood Moon Breeches grants your Minions a 3-7% chance to randomly apply Decrepify or Iron Maiden curses. Additionally, enemies afflicted by your curses will suffer a 100-150% increase in damage. The Breeches also provide the following affixes:
- +88.0-115.0% Overpower Damage
- +26.0-35.0% Curse Duration
- +638-873 Maximum Life
- +2-4 to Curse Skills
